JOB SUMMARY

The role of the Palliative Care Nurse Practitioner (PCNP) at Elite Home Health & Hospice is an instrumental position  supporting the organizations mission of providing life changing service.  We do this by helping our  patients as they navigate the complex medical system.  All PCNS’s at Elite are  responsible for providing competent, cost-effective, timely, and high quality health care services that are  within their scope of practice. 

DUTIES & RESPONSIBILITIES

1.    Under the supervision of the Director of Business Development, effectively deliver the clinical responsibilities and associated operational responsibilities. 

2.    Maintain performance and productivity standards, operational efficiency, excellent customer 
service and best practice standards.  

3.   Treat acute and routine medical conditions via in person, telephone call or telehealth across all 
settings (e.g. SNF, ALF and house call visits)

4.   Demonstrate critical thinking, expert care, and best practice standards in the clinical decision-
making process. 

5.   Provide care for patients from initial history taking, clinical assessment, diagnosis, prescribin
medications and treatments, providing education, and evaluation of care. 

6.   Maintain compliance with the established standards of practice of medicine for Nurse in the state of practice. 

7.   Document findings and actions appropriately in the patient’s medical record. 

8.   Maintain patient confidentiality at all times. 

9.   Participate in professional development activities and maintain professional affiliations and CME’s. 

10. Maintain safe work environment for self, patient, family, and staff by using appropriate PPE

11. Work collaboratively with Elite team members and community partners such as ALF and SNF staff
members, patient family members and home health and hospice team members. 

12. Represent Elite in a professional manner and strive to promote a cooperative atmosphere at all times
by acting with tact and diplomacy and maintaining a positive attitude. 

13. Maintain professionalism at all times by being dependable, autonomous, and responsible.   

14. Prioritize workload and utilize work time properly and productively. 

15. Be humble and possess the ability to accept criticism and/or discipline.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

Must have graduated from an approved school of nursing and be licensed in the state where practicing.

Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.

Maintains a current CPR certification.

Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Self-directed and able to work with minimal supervision.

Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.

Shows ability to organize and prioritize workload independently.

Management experience not required.  Responsible for supervising home health aides.

Prolonged or considerable walking or standing.  Able to lift, position, or transfer patients.  Able to lift supplies and equipment.  Considerable reaching, stooping, bending, kneeling, or crouching.  Visual acuity and hearing to perform required nursing skills.

Wage: $52 - $60 per hour
